Opportunities for Improvement

Monitoring core measure data allows our caregivers to quickly respond when the data deviates from best practice. Feedback is given to doctors and staff so that changes can be made to processes and practice.

Heart Attack Patients Given Aspirin at Discharge

What is the problem? There were only two opportunities to meet this measure in the past 12 months and one of them did not have aspirin prescribed.

How are we fixing it?  We are working with physicians to remind them to document the reasons why they choose not to comply with the recommended guidelines for certain patients.
